1. A valid original passport with a validity of at least 6 months with 2 blank visa pages
2. A completed evisa application form with signature
3. A digital copy of one recent passport sized colour photo
4. A clear digital copy of the passport bio page
5. A digital copy of the detailed flight itinerary showing applicant's full name
6. A digital copy of the Hotel reservation or letter of invitation from family/friend in Chile. The letter must
be signed before a Chilean Notary with the full details/contact of inviter
7. A digital copy of the proof of income in form of bank statements (last 3 statements)
8. Travel permit for minor (if applicable)
Note when completing the visa application form, please clearly complete the form using blue or black pen.
